By signing up for, or signing my child up for City Theatre's Company's Audition Preparation Dancing and Acting Workshop, I acknowledge and agree to the following:

-I am responsible for providing myself or my child with water in a non-glass water bottle, and with snacks that they may need to eat during the breaks between each session, should they want one.

- I understand that street shoes may not be worn in the dance studios and that I need to wear dance shoes, otherwise I or my child will be asked to be barefoot/in socks only for the duration of the workshop.

-I understand that City Theatre Company is not liable for any injuries that may occur during the camp. While the safety of students is City Theatre's number one priority, and we will never put students in unsafe or potentially dangerous situations, accidental trips and falls can happen, and if they result in any injury, a parent will be contacted immediately (if applicable) and City Theatre will not be held liable.

-I understand and agree to City Theatre's cancellation policy: 

If I need to cancel my enrollment, meaning that I or my child no longer is able to or wants to participate in our workshop, I will email or call City Theatre Company 7 days before the day of the workshop (On or before May 4th, 2024). If I cancel later than 7 days before the workshop, I am responsible for 50% of the total cost of all workshops I enrolled for. For example, if I sign up for the full workshop, and cancel with less than 7 days' notice, I will be refunded $85 of the total cost of $170.
